You may request the birth certificate, if yielded negative results on our database, you will receive a No Record Certificate.

Please be advised that our CRS Outlets DO NOT accept walk-in clients and only clients with prior appointment can be accommodated. To secure an appointment, please proceed to appointment.psa.gov.ph.

Customers may also file their requests for copies of their civil registry documents through the following:
