born around 1944 or 1945 in Pangasinan.

Thank you for your request dated Oct 12, 2022 02:05:57 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Certificate of Live Birth.

Your FOI request is approved. 

Please be advised that we are not authorized to verify a specific record due to RA10173 or the Data Privacy Act of 2012.

Also, we do not have records of civil registry documents from years 1945 and below.

Should you wish to know the availability of any Civil Registry Document, you may request for a copy thru the following channels:

1. Walk-in by Appointment via (appointment.psa.gov.ph)  at any PSA Serbilis Outlet
2. Online via www.psaserbilis.com.ph or www.psahelpline.ph
3. SM Business Centers.

Or you may try to verify the document in the Local Civil Registry Office where the document was registered or through the National Archives of the Philippines.
