To whom it may concern,

Greetings!

I would like to ask if I can get a replacement for my lost National ID Card because I need it for school and other offices. Due to some unfortunate circumstances, I lost my wallet, inside of it was my ID, cash, and a few personal items.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
