Hi, good day. 

I am Corazon Dalayoan and I’m humbly seeking for your assistance and request your good office to help me retrieve my grandfather’s birth certificate and death certificate for the purposes of connecting his direct heirs. 

Here are the following details:
ROQUE DALAYOAN GONZALES
DEATH: between JANUARY 1,1910 to DECEMBER 31, 1920
MOTHER: JOAQUINA DALAYOAN
FATHER: RAMOS GONZALES

Thank you for your request dated Mar 10, 2023 07:39:36 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Death certificate.

Your FOI request is approved. 

In reference to your concern, we regret to inform you that our civil registry certificates available are only reference years 1945 and above.

For your concern, you may try to coordinate with the National Archives of the Philippines to verify the death certificate. Or if not available, you may try to coordinate with the Local Civil Registry Office in the city/municipality of the death occurrence.
