Good afternoon,
I would like to follow up on my request for a correction on my Birth Certificate with the following:

 a. From first name: Remaltn M. Mondigo to Remalyn M. Mondigo.
b. The maiden name of my mother: Pacita M. Mondigo to Pacita B. Moral.

I already filed a petition for correction of a clerical error under the provisions of the amended RA 9048 (amended by RA No. 10172).  The Municipality of Paranas, Samar, local registrar processed my documents and sent a copy via LBC to PSA in 2018.
