Hi! 

I am an authorized representative requesting for a copy of the negative PSA registration in behalf of the minor Jareld Perez Parreño. Such document is being requested for the purpose of his late PSA registration, to be used for enrolment purposes.

We regret to inform you that we do not process requests of civil registry documents through this portal.

Please be advised that our CRS Outlets DO NOT accept walk-in clients and only clients with prior appointment can be accommodated. To secure an appointment, please proceed to appointment.psa.gov.ph.
