Hindi ko po alam kung anong tamang hingiiin. Kung Death Cert or Authentication or CDLI.  Nais ko lang po sana malaman ang taong hinahanap namin ay kung patay na po ba o buhay pa.

Your FOI request is approved. 

You may request copy of the death certificate. 

However, please be informed that we are not authorized to verify a specific record due to RA10173 or the Data Privacy Act of 2012.

Should you wish to know the availability of any Civil Registry Document, you may request for a copy through the following channels:

1. Walk-in by Appointment via (appointment.psa.gov.ph)  at any PSA Serbilis Outlet
2. Online via www.psaserbilis.com.ph or www.psahelpline.ph
3. SM Business Centers.

Or you may try to request for a manual verification in the Local Civil Registry Office of city/municipality where the death record was registered.
