Dear Mary Eileen Kalaw,

You asked for Birth Certificate.

Your FOI request is approved.

Please be informed that the correction of clerical errors, except for age, nationality, and legitimacy status, can be done by filing a petition for correction under the provisions of Republic Acts 9048 and 10172, or the Clerical Error Law. You may pursue the petition for correction at the Local Civil Registry Office (LCRO), where the birth certificate was originally registered. Please seek the advice of the LCRO on the details of the procedure, the forms to be accomplished, and the supporting documents that need to be submitted.

The usual requirements are baptismal certificate, voter's affidavit, employment/GSIS/SSS record, driver's license, and other documents that will support the correction being requested.

The filing fee for correction of a clerical error at the Local Civil Registry Office (LCRO) pursuant to Republic Act 9048 is One Thousand Pesos (Php1000) while for change of the first name, it is Three Thousand Pesos (Php3000) plus the cost of publication in a newspaper of general circulation. An additional Five Hundred Pesos (Php500) shall be collected by the accepting LCRO for migrant petitioners.
