I just want to inform your office that due to my carelessness I lost my National ID last Thursday, I do not know where I left it,,, I just founded that it already lost when I needed it.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
