I forgot when I lost it.

Greetings!

Thank you for your request dated Nov 03, 2023 09:35:41 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Lost PhilSys Transaction Slip.

Your FOI request is approved. 

Currently, the transaction reference number (TRN) is required to check if you have been issued your PhilSys Number (PSN) and are eligible to claim your ePhilID. We seek your understanding of this limitation and advise you to wait for the delivery of your physical card.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
