Hi! I would like to request another copy of National ID due to the first copy of my National ID was stolen from me. May I ask what are the requirements and how much would be the fee? Thank you.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
