Hi I would like to ask an assistance from you if I can request a copy of Philsys Transaction No. since it was almost 3 years already when I registered to Philsys. And also I lost the TRN slip of my registration together with phone last year, that's why it's very hard for me to track my National Id. I badly needed it for me to be able to get a passport since almost all of the government IDs were suspended for operation. Even ePhilID only if you can assist me it is a  very big help for me. Thanks,

Your FOI request is approved.

Currently, the transaction reference number (TRN) is required to check if you have been issued your PhilSys Number (PSN) and are eligible to claim your ePhilID.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
