Hello, I would like to request my PhilSys ID's TRN. I no longer have access to my phone number registered for my national/philsys ID.

I also no longer have access to my stub for claiming my PhilSys ID since all of my legal documents are no longer available. 

I would like to request for your help since I was only advised by the employees of PSA to wait for my PhilSys ID to be delivered to our house but it has been over a year. I registered year 2021.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
