Hi, I hope you're doing great. 

I would to ask for an assistance regarding the correction of my National ID information

My Full name is supposed to be 

Full name: Mary Joy

Middle Name:  De La Peña 

Last name: Ignacio 

what I received is Mary Joy De La Peña they put my middle name as my last name and in my ID i don't have a middle name. 

Please help me on this matter. Thank you.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
