Hello! I would like to request scheduling for delivery of my national ID, I applied around the year 2021. I have already completed all the required steps. 

I have the transaction number 72100207510558420211109073415 and my name is Mary Rose Sanchez Agusan. 

My live-in partner and my brother had applied on the same day and they already got their national ID except me. Therefore, I am humbly requesting the delivery of my national ID at 112 Don Placido Campos Avenue, San Agustin 2, Dasmariñas, Cavite 4114

Thank you for your request dated Dec 28, 2023 09:17:32 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Request Letter for my National ID delivery to my New Address.

Your FOI request is approved. 

We would like to inform you that the address for delivery of the PhilID cannot be updated; the Post Office will attempt to deliver your PhilID to the address you provided during your Step 2 registration. You may authorize an individual from your previous address to receive your PhilID, or you may claim your PhilID at the Post Office. Please provide an authorization letter and a copy of your ID to your authorized representative.

To further assist you, kindly send your Transaction Reference Number or TRN printed on the transaction slip provided to you during your Step 2 registration to our PhilSys Registry Office through email address info@philsys.gov.ph. You may also track your PhilID using your TRN at https://tracking.phlpost.gov.ph/.

Meanwhile, you may also check if you may claim your ePhilID at https://appt.philsys.gov.ph. The ePhilID has the same functionality and validity as the physical card.
