I recently lost my National ID and I would like to report it as stolen/lost. May I also know how can I request for a replacement and what are the requirements needed to be submitted. Thanks and regards.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
