I lost the transaction slip given for me to claim my national ID. I would like to request for an assistance on how to track or claim my national ID without it?

Greetings!

Thank you for your request dated Sep 22, 2023 07:48:09 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for National ID.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN) to get an ePhilID, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
