Hi there, 

Hope this email finds you well.

I am reaching out as it has been more than a year and my national ID has still not been delivered yet. Also due to unforeseen circumstances, I lost my transaction slip which is why I cannot track my national ID. I have checked online and was advised to send an email which is why I have reached out to you. I do hope I get a response from you soon with any updates about this matter.

Thank you for your request dated Oct 07, 2023 03:43:31 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Lost transaction slip for the national ID and has not receive it for more than 2 years now..

Your FOI request is approved.

Currently, the transaction reference number (TRN) is required to check if you have been issued your PhilSys Number (PSN) and are eligible to claim your ePhilID. We seek your understanding of this limitation and advise you to wait for the delivery of your physical card.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
