Greetings! 
I would like to request a replacement for my lost national id since it's really important for me for job employment. 
I never intended to lose it. But it happened already 

Please tell me what requirements I need to provide enable for me to get new one .
Thank you so much or would be a big help for me and my career.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
