Hi! I would like to request for a new National ID since mine was lost over a month ago.

I'm already an employee for a year now and it would be really helpful to have this ID for documentation and validation purposes.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
