Hi! I would like to request to remove middle name in my National I.D. The name listed is Rian Del Mundo Aycardo (my fathers surname) since I don't have a PSA that time. But my present name is my maiden name Rian Del Mundo cause I don't have a maiden name. I will be needing the correct card for my vaxcert for travel purpose. Kindly let me know how this issue can be resolved.

Thank you for your request dated Jan 12, 2023 03:36:14 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for removing of middle name on my national i.d.

Your FOI request is approved. 

We would like to inform you that the request to change/revert/edit any details  that was provided during the Step 2 registration is not yet available for processing.

Kindly wait for the announcement in regard to the updating or renewal of your PhilID.

Be reminded that you do not need to register again, since you will be provided with your PhilSys Number or permanent and unique identification number upon completing your Step 2 registration process.

You may also track your PhilID using the transaction number printed on the transaction slip provided to you during your Step 2 registration. Kindly visit https://tracking.phlpost.gov.ph/ to check the status of delivery.
