Hi I would like to request for replacement of my National ID since it was lost awhile ago because my wallet was pickpocket while I'm riding a bus. Aside from national ID my other IDs was also in the wallet: PhilHealth ID, TIN ID, My Dad's SSS ID and also his national ID

I don't have any valid IDs anymore and I hope my request will be granted. Thanks

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
