Unfortunately,because it has been a long time sine the date of registration,i lost the claim stub.

Thank you for your request dated Dec 22, 2023 05:52:12 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Philippine National ID claim stub.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN), please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
