I hope this message finds you well. I am here to report my lost National ID, and would like to request a replacement for it. I hope this letter would reach to you, and approve my request because I really need it for my identification for school purposes. Thank you and God bless.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
