Dear Ma'am/Sir, 

          I would like to ask if I can get a replacement for my lost National ID Card. Because, I really need it to my upcoming board exam Feb. 2024. I already have affidavit of lost for my national id.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
