To whom it may concern,

I would like to inquire about how can I get a replacement for my lost National ID Card. I badly need it for school, other offices and personal important transactions.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
