Hi I'd lost my receipt. How can i claim my national id? Can i request a copy of my tracking number? I live in Angono and my full name is Sonny Rabe Valencia the id was requested or registered at sm taytay rizal.

Greetings!

Thank you for your request dated Jan 17, 2024 07:07:32 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Philippines National ID.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

Moreover, in reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
