Hi, I would like to request advice about my transaction slip of PhilSys because I lost it.

Greetings!

Thank you for your request dated Jan 15, 2024 05:21:29 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for I lost my transaction slip of PhilSys ID.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
