Hi! I would like to request for the transaction number of my registration for National ID. I no longer have the transaction stub as well as I no longer have access to my old phone number as registered.

You asked for Lost Transaction Number/Stub for claiming National ID.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
