Hi I would like to request to fix my national id because there's a mistake regarding in my birth date, instead of July 09,2005 it should be July 19, 2005 the (1) is missing and i would like to include my blood type AB+.

We apologize for the inconvenience caused. As of now, we encourage you to keep your PhilID safe and wait for further announcements on the process of updating incorrect information printed in your PhilIDs.  

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
