I AM REQUESTING A COPY OF PSA NEGATIVE RESULT FOR EACH OF MY CHILDREN AND I WILL NEED IT AS SOON AS POSSIBLE. URGENTLY NEEDED.

My name is Kenneth C. De Guzman and my birthday is on August 15, 1991. My address is Purok 1 Barangay Capoocan Calbayog City Samar. Below are the details of my children.

Please be advised that our CRS Outlets DO NOT accept walk-in clients and only clients with prior appointment can be accommodated. To secure an appointment, please proceed to appointment.psa.gov.ph.
