To whom it may concern,
I would like to request for a replacement of my NATIONAL ID, upon receiving it there was my photo attached to it, but as time goes by, my photo simply went gone without even knowing the reason why.
I hope this can be resolved as soon as possible.

Please bring a valid ID and your transaction slip (if still available). You will be requested to fill out a Request Form at your chosen PSA Office upon surrendering.
