Dear Sir/Madam,
I am writing to request your assistance in retrieving a lost claim stub for my National ID. I had entrusted my National ID claim stub for a long time to the staff of StayBGC Dorm located at 3692 Gabong St., Pinagkaisahan, Makati City with a phone number +639178648876. Unfortunately, when I approached Maam Tin (staff at reception area) to collect the claim stub yesterday, they informed me that it has been misplaced or lost.
