Pleasant day!
I would like to request for my mom's birth record if registered for I will need it to correct my middle name. If none, I would like to know the process and requirements for late registration.

Dear April Rhea Cortez,

Greetings!

Thank you for your request dated Oct 27, 2022 10:46:32 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Birth Certificate.

Your FOI request is approved. 

Please coordinate with the Local Civil Registry Office (LCRO) where the event took place for the delayed registration of birth/marriage/death. You may also file for Out-of-Town Delayed Registration of Birth/Marriage/Death at the Local Civil Registry Office where you are currently residing. The LCRO will provide you with assistance on the details of the procedure, the forms to be accomplished, and the supporting documents that need to be submitted.

Kindly refer to some of the following requirements.

A. For persons less than eighteen (18) years old:

1. Four (4) copies of the Certificate of Live Birth duly accomplished and signed by the proper parties.

2. Affidavit of Delayed Registration (at the back of the Certificate of Live Birth) signed by the father, mother or guardian, or the child him/herself, if he is of understanding capacity.

3. Any two of the following documentary evidence which may show the name of the child, date and place of birth, and the names of the parents:

*Baptismal certificate
*School Records
*Income Tax of Parents
*Insurance Policy
*Medical Records
*and others such as Barangay Captain's Certification

4. Affidavit of two (2) disinterested persons who might have witnessed or have known about the birth of the Child; and

5. A sworn statement of the present whereabouts of the mother. If the person is seeking late registration of an illegitimate child is not the mother.

B. For person eighteen (18) years old or above:

1. All requirements for a child who is less that eighteen years old; and
2. Certificate of Marriage if Married
