Dear Emmanuel Colot,

Greetings!

Thank you for your request dated Jan 08, 2024 04:19:00 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Copy of National ID.

Your FOI request is approved.

Kindly wait for the announcement in our official channels for the process of replacement of lost/stolen PhilIDs.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.

However, requests for issuance or replacement of ePhilID can be made personally at the PhilSys registration center.

We encourage you to safe keep and use the ePhilID issued in the PhilSys registration center. Reproduced copies may have lower print quality and affect the QR code image and its scanning.

Photocopied ePhilIDs are acceptable only if the QR code is still readable via PhilSys Check.

For other concerns, you may reach PhilSys Registry Office thru email at info@philsys.gov.ph, Hotline 1388, and https://facebook.com/PSAPhilSysOfficial.

We would love to hear your Feedback so we can improve our services. Kindly fill out the PSA Customer feedback form from this link: https://tinyurl.com/OnlineCSSForm.

Thank you.

Respectfully,

Rene C. Mendoza
