Goodday,

         I would like to request for a new National ID, since ive lost my National ID for almost 5months now i have tried everything to recover but i failed, this is my last hope, i need it for my school and gcash account. Hoping your deep consideartion on my request, Thank you.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
