Greetings!

Thank you for your request dated Sep 15, 2023 08:32:36 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Lost transaction slip for national id.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN) for lost transaction slip in order to get ePhilID, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
