Good Morning! I would like to request information regarding the marriage of JULIUS HERNANDEZ ESCUTIN and MA. THERESA VILLANUEVA BAXTER  on 14 April 1977 by HON. CORNELIO G. LAZARO, City Judge of Iloilo City.

1) Whether or not there is an annotation on the Marriage Certificate or any information on record that the subject marriage above has been annulled

For your specific concern, please coordinate with our Civil Registry Service through email addresses crs.qvu1@psa.gov.ph,crs.qvu2@psa.gov.ph, crs.qvu3@psa.gov.ph by providing them the following scanned documents:
     1) Certified True/photocopy of the Court Decision;
     2) Certified True/photocopy of the Certificate of Finality;
     3) Certificate of Registration of the Court Decision;
     4) Certificate of Authenticity of the Court Decision;
     5) Certificate of Marriage Without annotation; and
     6) Certificate of Marriage With annotation.
