Hello! I did not receive my PhilSys ID yet (or I haven't yet issued one as I did not receive it even after more than a year of registration for it) but I received a copy of my ePhilID but I lost the piece of paper and now I need it to apply for a TIN number. I also was not able to take note of my ID number.  I would like to request a new copy of it if possible.

You asked for Lost ePhilID (PhilSys ID number).

Your FOI request is approved. 

Requests for issuance or replacement of ePhilID can be made personally at the PhilSys registration center. 

We encourage you to safe keep and use the ePhilID issued in the PhilSys registration center. Reproduced copies may have lower print quality and affect the QR code image and its scanning.

Photocopied ePhilIDs are acceptable only if the QR code is still readable via PhilSys Check.
