Your FOI request is approved. We regret that we do not process online verification or provide appointments concerning civil registry documents through this email.

Should you wish to know the availability of any Civil Registry Document, you may request for a copy thru the following channels:

1. Walk-in by Appointment via (appointment.psa.gov.ph)  at any PSA Serbilis Outlet

2. Online via www.psaserbilis.com.ph or www.psahelpline.ph
