Good morning, I just want to ask if you have a record of my Birth Certificate, so when I suggest a copy of it I'll know if I'm registered and it will lessen the hassle of me commuting there and getting the documents needed. If ever I’m not registered, can you tell me what are the procedures and needed documents in order for me to be registered?

Thank you for your request dated Jul 12, 2023 11:12:36 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Request for Records of Birth Certificate.

Your FOI request is approved. 

Please be informed that we are not authorized to verify a specific record due to RA10173 or the Data Privacy Act of 2012.
 
Aside from that, Civil Registry documents are available four to six months after the date of registration.

Should you wish to know the availability of any Civil Registry Document, you may request for a copy through the following channels:

1. Walk-in by Appointment via (appointment.psa.gov.ph)  at any PSA Serbilis Outlet
2. Online via www.psaserbilis.com.ph or www.psahelpline.ph
3. SM Business Centers.
