Hi! I would like to request to get my tracking number for my Philippines National ID as I have lost the transaction slip when I registered. It would really help as it has been 2 years since I registered for it.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
