Hi,I lost my tracking number of my nationa Id.and I don't what my number is or the details providesd there and I am 18 yrs old I don't have any valid id yet except for that nationa i.d I lost.and now I am requesting if i  could get another slip.

You asked for National id transaction slip.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
