Greetings!

Thank you for your request dated Sep 14, 2023 10:25:28 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for PhilID Claim Stub Copy.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

Meanwhile, you may also check if you may claim your ePhilID at https://appt.philsys.gov.ph. The ePhilID has the same functionality and validity as the physical card.

However, if you are requesting for transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
