I would like to get a replacement for my lost National ID because I need that for the filing of incoming Board Exam at Professional Regulations Commission (PRC) this November.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
