Greetings!

Thank you for your request dated Nov 07, 2023 01:41:20 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Request for a claming Stub copy of the National ID.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
