Good Day!

I am Ebitte Dosado from Iloilo Doctor's College, 

We are currently working on a thesis proposal to compare the causes and number of deaths. I would like to request for:

1. Death certificate of an anonymized individual in Western Visayas in the year 2019 and 2020.

I would like to ask if you can release the cause of death as SARS-COV infection. In addition, I would like to request for a complete cause of death from immediate antecedent and underlying cause together with its other significant conditions.

We do not provide specific data like date of birth, place or birth, occupation etc. All our data are anonymized. We can only provide you number of deaths, cause of deaths, sex, or place of occurence.

Should you need the data, please send us a letter request addressed to Usec. Dennis S. Mapa indicating your specific request and the purpose why you need the data and send this to ons@psa.gov.ph, copy us at info@psa.gov.ph.

Once approved, we will endorse your request to Vital Statistics Division for their consideration.
