Hi! I would like to request a copy of my National ID Transaction Slip and/or request the status of my National ID since my transaction slip was lost and my National ID is still not delivered to my home address.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
