Good day! 

I want to ask for some assistance concerning my National ID Tracking Number. The facility (SM Daet) I registered in failed to provide a tracking slip and merely advised me to wait. It has been a year already, and I would like to know the progress of my ID.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
