Greetings!

Thank you for your request dated Dec 21, 2023 12:22:19 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Ntional ID claim stub copy.

Your FOI request is approved. 

Please be informed that the Transaction Slip has been provided during your Step 2 registration at the Philsys Registration Center. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery. In cases that you cannot personally receive your PhilID, you may leave an authorization letter with a photocopy of your valid ID and your transaction slip to the person you authorized to receive your PhilID. 

The Post Office will attempt to deliver your PhilID to the address you provided during your Step 2 registration. You may authorize an individual to receive your PhilID, or you may claim your PhilID at the Post Office. Please provide an authorization letter and a copy of your ID to your authorized representative.
